Privacy at Pharmacy Pathway
This notice explains how Pharmacy Pathway handles information when you use this website and its scheduling services.

Who operates Pharmacy Pathway
Pharmacy Pathway is owned and operated by Sloan's Pharmacy. This notice applies to the Pharmacy Pathway website and services controlled by Sloan's Pharmacy. A participating pharmacy or clinic organizer may provide an additional notice for its own activities.
Information we collect
The public homepage contains no registration form and does not ask for patient or health information. Our hosting, security, and operational services may create standard technical records such as an IP address, browser and device information, requested pages, timestamps, and security events.
Dedicated scheduling pages may request contact, appointment, eligibility, insurance, consent, and health-related information needed to provide the selected service. The fields presented on a scheduling page determine the information requested for that service.
How we use information
We use information to operate and secure Pharmacy Pathway; provide scheduling, confirmations, updates, and reporting; support participating pharmacies and clinic organizers; respond to authorized requests; prevent misuse; and meet legal, regulatory, and recordkeeping obligations.
Cookies and sign-in
Pharmacy Pathway uses cookies and similar browser storage when necessary for secure sign-in, session continuity, request protection, and core site operation. Blocking required cookies may prevent authenticated features from working. We do not currently use advertising cookies on the public homepage.
When information is shared
Information may be shared with the pharmacy or clinic involved in the service, with vendors that help us host, secure, communicate, and operate the platform, or when required to protect rights, safety, and service integrity or to comply with law. Vendors are expected to use information only for the services they provide to us and under applicable contractual safeguards.
Third-party services
Some registration workflows may use or link to third-party services, including Cognito Forms. Information submitted directly to a third party is also governed by that service's terms and privacy practices. Links to external websites do not mean Pharmacy Pathway controls those websites.
Security and retention
We use administrative, technical, and physical safeguards designed to protect information. No internet service can guarantee absolute security. Information is retained only as long as reasonably needed for the purposes described here, applicable pharmacy operations, contractual requirements, dispute resolution, and legal or regulatory obligations.
Your choices
Use the secure options provided with your registration to review or manage it when available. Privacy questions and requests may be submitted through the Pharmacy Pathway contact page. Do not include patient or health information in the general operational email.
Children
The public website is not directed to children for independent use. A parent, guardian, or other authorized adult should complete any registration for a minor when permitted by the applicable service.
Changes to this notice
We may update this notice as Pharmacy Pathway evolves. The effective date above will be revised when changes are posted.
